SUPPLEMENTAL STAFFING SERVICES
CareMate Corporation provides the following:

• Registered Nurses (RN)
• Licensed Practical Nurses (LPN)
• Nursing Assistants / Registered (NA / R)
• Trained Medical Assistants (TMA)
• Personal Care Attendants (PCA)

CareMate Corporation has been in business since 1989. In these past 10 years, no long term care facility has been fined directly due to a staff of CareMate Corporation. We guarantee that our Nursing Assistants have the following:

• Are currently registered by the Minnesota and/or Wisconsin Nurse Aide Registry
• Has a current background check approved by the Department of Human Services and/or Department of Health and Family Services
• Has a minimum of 6 months experience
• Are eligible to work in the United States with proper documentation
• Has a current Mantoux
• Are covered by Professional Liability Insurance
• Are covered by Minnesota Worker's Compensation
• Has completed 12 (twelve) hours of In-Services each year
• Corporate Protective Liability Insurance

CareMate Corporation adheres to the Federal and State laws or statutes regarding necessary insurances that protect both our employees and clients.

Our nurses are covered under:

• Worker's Compensation Insurance
• Professional Liability Insurance
• Federal Unemployment Insurance
• Minnesota Unemployment Insurance
• Employment Dishonesty Bond (Protects all of our Home Health Clients from legitimate loss incurred by dishonest acts of our employees.)
• Upon request, CareMate Corporation will provide our client with certificates of insurance in that facility / agency name.

HOME HEALTH CARE PROVIDER

CareMate Home Health Care, Inc. provides the following services:

CareMate Home Health Care is a small, unique home health care agency which serves those needing health care in their own homes. Our nurses, aides, and therapists will visit patients at home and provide the care needed to make it possible for them to stay home, even while they are sick. This often prevents the need for hospitalization or a move to a nursing home.

Because we are small, we are able to provide exceptional care to each patient, responding quickly to any of the patient's needs. CareMate is unique because we are a diverse staff serving diverse patients including people from Hmong, Cambodian, Vietnamese, Laotian, African, African American, Native American, and Hispanic cultures. We provide interpreters for anyone not comfortable with the English language. Our staff represents several different cultures, so we are sensitive to individual needs and preferences unique to specific cultures. At the same time, we are well grounded in the American culture and appreciate the opportunity to serve all Minnesotans.

We provide services ordered by the Physician. Our nurses provide physical care and coordinate medications. They also coordinate all care, including use of Durable Medical Equipment and Medical Supplies. Our Home Health Aides provide patients with baths and other personal cares under the supervision of a skilled nurse.

Physical Therapists and Occupational Therapists work with patients at home to speed recovery from physical problems resulting from a disease or disability. The Speech Pathologist helps with communication problems. Social Workers will assist with applications for financial assistance or changes in residence and can refer the patient to community resources; they also counsel patients and family members regarding adjustment to the emotional effects of illness. Personal Care Attendants tend to many personal care and homemaking needs of the patient.

Our Homemakers will do basic cleaning of homes, laundry, meal preparation and grocery shopping. Chore service workers do heavier cleaning, both inside and outside the home. If the patient cannot stay alone and needs minimal nursing attention, we provide Companion. For physician-ordered lab work, our nurse draws the blood and delivers it to the laboratory.

Medicare, Medical Assistance (Medicaid), most Managed Care Providers, and the Veterans Administration all pay for health care provided in the patient's home with physician orders. We handle insurance or government agency paperwork for the patient; private pay is also accepted. Regular reports are sent to the physician regarding all aspects of the patient's home care.
